Before your first appointment you will be asked to fill out a medical history form. For your convenience, we have provided this form in English or French and they are both available from this website. Simply print off the form, fill in the information and bring it with you or fax it into our office before your visit. This helps ensure that before the dentist examines your mouth, she will be aware of any important medical factors that could affect your treatment.

Your first appointment is a two way process where your dentist will discuss your concerns about your mouth. In turn, your dentist will carry out an extensive examination and propose a treatment plan if needed.
